I/We declare and consent to the hospital providing medical services of sharing my personal health data - clinical, billing or administrative data of this particular visit to the EvenHealthcare Private Limited for the purpose of claim processing or general improvement of services provided to me by hospital or Even Healthcare.

I/We declare and consent to Even Healthcare Private Limited seeking medical information from any hospital or such similar provider, establishment, or clinic, who at anytime has attended to me in the past or present concerning anything which affects my physical and mental health, for the purpose of paying claims raised by such hospital under subscription of Even Plan.

I/We authorize Even Healthcare Private Limited to process, use, store, disclose and transfer any information, including medical information, as is made available by me, for the sole purpose of (i) confirming my subscription to the Even Plan and the provision of any services thereunder, (ii) any purposes contemplated under the Even Plan, and (iii) any activities related or incidental thereto, provided that such authorisation shall always be subject to applicable law in force.
